Resilient health systems begin with strong primary care and dependable referral pathways. Digital tools can support continuity, but only when they fit clinical workflows, protect patient rights and remain usable in low-connectivity environments.The proposed approach combines community-level access, responsible information exchange and investment in the health workforce. Technology is treated as an enabling layer rather than a substitute for trusted care.
